Treadmill vs. Overground Running: What the Science Says
Does treadmill running feel different from running outside? Explore how the two compare and what those differences mean for your routine.
Most running mechanics are comparable, with some differences
A treadmill run and an overground run are not mechanically identical, but the evidence does not support treating them as entirely different movements. A systematic review of 33 studies involving 494 participants found that most biomechanical outcomes were comparable between motorized treadmill and overground running.
The pooled results did identify differences. On a treadmill, the sagittal foot-ground angle at footstrike was lower, contact time was about five milliseconds longer, and vertical displacement was about 1.5 centimetres lower. Peak propulsive force was also slightly lower, by about 0.04 body weights. The certainty of evidence for these specific findings was low or very low, and muscle-activity findings conflicted across studies. Those qualifications matter when interpreting a small average difference as advice for an individual runner.
A lower measured peak propulsive force does not establish that the belt does the runner’s propulsion for them. Nor does it show that every overground run requires greater total muscular effort. The review compared biomechanical measurements; it did not determine why a particular runner might feel that pavement is harder, or establish a universal adjustment period when moving outdoors.
What a smaller joint-motion study adds
A 2013 study of 12 participants compared three-dimensional lower-limb motion at 4.0 metres per second on a treadmill and overground. Hip flexion at footstrike was about 12 degrees lower on the treadmill, and ankle excursion to its peak angle was 6.6 degrees smaller. Peak ankle eversion was 6.3 degrees greater.
These are joint-angle findings. They do not directly quantify hip-flexor effort, ankle-stabilizer loading, tissue damage, or injury risk. The study also tested a single speed, so its measured angular differences should not be assumed to apply identically to easy jogging or faster intervals. The larger review’s finding of broadly comparable mechanics helps put this small study’s differences in context.
Using the comparison in training
The practical lesson is to distinguish measurable differences from explanations the studies did not test. Treadmill data can inform an understanding of overground running, while details at footstrike and other specific measures may depend on the setting. Neither study demonstrates that a few outdoor sessions are enough for everyone to adapt, or that mixing indoor and outdoor runs prevents injury. If the conditions feel different, that experience is useful feedback, but these studies alone cannot identify its cause.
